All students in grades 6-12 who plan to participate in any sport or cheerleading must have their physical forms completed and signed by a healthcare provider with the credentials of MD, DO, DC, PA-C, or APRN and a parent or guardian on or before the first day of practice in order to participate. In addition to the physical form, students and parents must complete a KSHSAA Recommended Concussion & Head Injury Information Release Form. Students may pick up these forms in the high school office if they desire to use their personal family physician or clinic. Copies or other forms will not be accepted. Only approved forms will be accepted for participation. Athletes must have a completed current physical on file with the athletic director before the athlete can practice in the sport of choice.
